The Sized app needs to be activated for each theme separately. When you switch to a new theme, the app widget is not automatically carried over.
To fix this, go to the Installation page in the Sized app, select your new active theme, and follow the installation steps to add the Sized widget. Your size charts and their product assignments are preserved — you only need to re-add the widget to the new theme.
